The Personal Planning System (PPS)
assists agencies as they provide person-centered planning services to
consumers. The PPS gives QMRP's or QDDP's more time to work with
individuals and less time doing paper work.
The PPS assists with gathering the information necessary to develop a vision for the future.
The Vision for the Future leads to the to establishment of personal
goals. Each personal goal then receives a combination of supports,
services and skill training in order to make the personal goal a
reality. The PPS produces the consumer's Individual Support Plan, and
assists with the progress reviews.
